Speech therapy billing services for speech-language pathologists

Speech therapy billing is coding and claiming a speech-language pathologist’s evaluations and treatment sessions under the plan of care and the modifiers payers require, as treatment codes move from per-session to timed in 2027.

Individual treatment moves from 92507 to ten timed codes by clinical area on January 1, 2027.

Individual treatment: which code?

Changes in 2027

92507 is replaced by ten timed codes by clinical area.

  • 92658first 30 min
  • 92659each further 15

The new codes are timed, so the minutes documented for each clinical area decide what is billed. Each payer decides when it adopts them.

Why is SLP billing hard?

  • The main treatment code changes in 2027

Individual speech and language treatment is billed with 92507, once per session, through December 31, 2026. From January 1, 2027 it is replaced by ten timed codes by clinical area (92654–92663), and each payer decides when it adopts them.

  • Untimed and timed codes side by side

Swallowing treatment (92526) and the evaluations are billed once per session, while cognitive treatment (97129, 97130) is timed, so one practice counts units more than one way.

  • Evaluations can’t all be combined

Each evaluation code has a set scope: 92523 includes the speech sound evaluation in 92522, so the two aren’t billed together, while fluency (92521), voice (92524) and swallowing (92610) are separate codes.

  • The plan of care has deadlines

As in physical therapy, Medicare wants the plan of care signed by the physician or NPP within 30 days of the first treatment, recertified at least every 90 days, and a progress report at least every 10 treatment days.

  • A threshold shared with PT

SLP services carry the GN modifier, and speech therapy shares Medicare’s yearly therapy threshold with physical therapy, so a patient may need the KX modifier sooner if they also had PT that year.

Speech therapy billing questions

What is replacing CPT 92507?
From January 1, 2027, ten timed codes replace 92507 for individual treatment, one pair per clinical area: fluency (92654, 92655), speech sound production (92656, 92657), language (92658, 92659), speech sound and language combined (92660, 92661), and voice, upper airway and resonance (92662, 92663). Each pair is a base code for the first 30 minutes and an add-on for each further 15. 92507 stays in use through December 31, 2026.
Does the 8-minute rule apply to speech therapy?
Only to timed codes. Through 2026, 92507, 92526 and the evaluations are untimed and billed once per session, while timed codes such as cognitive function intervention (97129 for the first 15 minutes, 97130 for each further 15) follow Medicare’s rules for time. From 2027 the codes that replace 92507 are timed too, so the minutes documented for each clinical area decide what is billed.
What CPT codes are used for speech therapy evaluations?
92521 for fluency, 92522 for speech sound production, 92523 for speech sound production with language, 92524 for voice and resonance, and 92610 for swallowing. 92523 includes what 92522 covers, so the two aren’t billed together.
What is the GN modifier?
GN marks a service given under a speech-language pathology plan of care. Medicare requires it on every SLP service, and many other payers ask for it too.
